You, the customer, grant full production and editorial control of all aspects of editing and creating your DVD video to Family Legacy Video, Inc. (hereafter referred to as "FLV"). FLV agrees to work with you to create a DVD video that meets your specifications. FLV agrees to make every provision possible to incorporate your suggestions and ideas into your DVD video.

Please do not send us irreplaceable items. We cannot be responsible for items lost in the mail. Please make copies of any one-of-a-kind items and send the copies to FLV.

Specific photograph colors may not appear to be the same on screen; therefore, FLV is not responsible for photographs that appear to be slightly darker or lighter than the original prints. However, all attempts will be made to produce an accurate representation of each photo.

You are responsible for numbering each photograph and for specifying the order in which they are to appear in your Music 'n' Memories DVD™. This can be done by writing on the back of each photo in light pencil. If sending photo files on a disc, send a separate sheet listing the file names in the order in which they should appear. FLV accepts jpg, gif and tif file formats only.

FLV agrees to make every attempt possible to verify all information given by you in the online order form. It is your responsibility, however, to ensure that all information provided on the order form is correct prior to submission. All selections are final once customer has turned in a completed order form unless otherwise agreed upon with FLV. In the event that the incorrect information is included in the video as a result of incorrect information provided in the order form (e.g. misspellings, incorrect information, music, etc.), it is your financial responsibility to purchase an additional copy with the correct information and to pay shipping to and from FLV.

PAYMENT
Full payment is due along with your photos and materials. Work will not commence until complete payment is received and confirmed. Payments can be made by check, money order or credit card. Any bank charges incurred due to returned checks will be charged to you along with a $40 insufficient funds fee. All personal checks must clear before work commences, with no exceptions.

COPYRIGHT
Your video is a work for hire. The copyright belongs to you. However, you grant FLV the right to use your completed video for advertising, marketing, display or any other appropriate purposes.

All music used in all FLV videos is copyrighted. You cannot use the music contained in your video for any other purposes.

EXTENUATING CIRCUMSTANCES
The due performance of this agreement is subject to alteration or cancellation by FLV owing to any cause beyond it's control. In the event that a video is damaged during shipping, FLV will ship a new video to you (at no additional cost) upon the return of the damaged video.
